We are seeking an experienced and motivated Nurse Practitioner (NP) to provide high-quality medical care within Skilled Nursing Facilities (SNFs). Responsibilities

  • Conduct comprehensive patient evaluations and assessments
  • Manage acute and chronic medical conditions
  • Perform routine patient rounds within Skilled Nursing Facilities
  • Develop and implement treatment plans
  • Order, review, and interpret diagnostic testing and laboratory results
  • Collaborate with physicians, nursing staff, therapists, and interdisciplinary care teams
  • Maintain accurate and timely EMR documentation
  • Participate in care planning and quality improvement initiatives

Qualifications

Required

  • Active Nurse Practitioner license
  • DEA license (or ability to obtain)
  • Experience in Skilled Nursing, Post-Acute Care, Long-Term Care, Geriatrics, Internal Medicine, Family Practice, or Hospital Medicine
  • Strong clinical assessment and diagnostic skills
  • Ability to work independently while collaborating with a multidisciplinary team

Preferred

  • Previous SNF or Post-Acute Care experience an an RN or Nurse Practitioner
  • Experience managing medically complex patients
  • Strong communication and leadership skills

Compensation & Benefits

Compensation

  • $150,000–$180,000+ annually
