Description

Jaffrey Family Medicine, a practice of Monadnock Community Hospital, is seeking a compassionate and patient-centered Nurse Practitioner (ARNP) to join our dedicated primary care team. This is an excellent opportunity to practice independently while collaborating closely with experienced physicians and healthcare professionals who share a commitment to delivering exceptional care to our community.

At Jaffrey Family Medicine, you will build lasting relationships with patients of all ages, promote wellness, manage chronic conditions, and help improve health outcomes in a welcoming and supportive environment.

What You'll Do

As a Nurse Practitioner, you will provide comprehensive primary care services, including:

  • Conducting complete patient histories and physical examinations
  • Assessing, diagnosing, and managing acute and chronic medical conditions
  • Developing individualized treatment plans and coordinating patient care
  • Ordering and interpreting laboratory tests and diagnostic studies
  • Prescribing treatments and monitoring patient progress
  • Providing preventive care and patient health education
  • Collaborating with physicians and other healthcare providers to ensure high-quality, coordinated care
  • Making appropriate referrals to specialists and community resources
  • Triaging patient calls and providing clinical consultation when appropriate
  • Monitoring and managing patient care between visits
  • Participating in community outreach and health education initiatives
  • Maintaining accurate and timely documentation within the electronic medical record

What We're Looking For

Required Qualifications

  • Graduate of an accredited School of Nursing
  • Successful completion of an approved Nurse Practitioner program
  • Current New Hampshire Advanced Registered Nurse Practitioner (ARNP) license
  • Current DEA registration
  • Eligibility for professional liability insurance coverage through MCH

Preferred Qualifications

  • One or more years of experience in a clinical or ambulatory practice setting
