Job Summary:

LIFE Geisinger is seeking an experienced Nurse Practitioner. Experience working with elderly, nursing home level of care, patient population is preferred.

Job Duties:

LIFE Geisinger South Wilkes-Barre provides a full spectrum of healthcare services designed for older adults, including specialized senior care, daily meals, social activities, and more. Our program helps individuals age 55 and older maintain their independence and remain in their homes while receiving comprehensive medical and supportive services. It truly offers the best of both worlds.

LIFE Geisinger features a unique, team-based model of care for older adults with complex medical needs. Physicians work closely with advanced practitioners, pharmacists, therapists, social workers, care coordinators, and families to deliver comprehensive, personalized, relationship-centered care.

• Support acute visits, post hospitalization, ED and patient fall follow-ups
• Assesses and establishes a presumptive diagnosis and implements treatment plans for their patient population.
• Makes accurate and complete entries into the electronic medical record; orders and interprets diagnostic testing and consults as appropriate and prescribes appropriate medical therapy utilizing evidenced based practice.
• Prescribes, adjusts or discontinues medications or therapies as appropriate.
• Evaluates and reports to collaborating physician any circumstances or approaches to medical management or test results that present a significant risk to patient's health or well-being.
• Performs procedures within the scope of practice of the nurse practitioner license.
• Communicates with collaborative physician, patient, PCP and referring staff through telephone or other means of correspondence.
• Instills confidence in patient acting as their advocate by providing safe, ethical care and health education to the patient and their family in a language that they can understand.
• Interprets and follow policies and guidelines to define procedures and meet goals and deadlines.

• 30-minute appointment times for thoughtful, comprehensive care
• Mix of chronic disease management, acute visits, post-hospital follow-ups, and annual assessments
• Collaborative interdisciplinary care team providing extensive clinical support
• Occasional home visits and monthly skilled nursing facility visits
• One week of call per month with nurse triage managing first-call responsibilitiesSupport acute visits, post hospitalization, ED and patient fall follow-ups
