Nurse Practitioner (Home Healthcare Consultant)

About the Role:

We are looking for a skilled and compassionate Nurse Practitioner to join our home healthcare team. In this role, you’ll provide advanced care directly to patients in their homes, performing assessments, developing treatment plans, and managing ongoing care. The ideal candidate will be experienced in autonomous clinical decision-making and committed to delivering high-quality, patient-centered care.

Key Responsibilities:

• Perform comprehensive patient assessments, including physical exams and health histories.
• Diagnose, manage, and treat acute and chronic conditions, working closely with patients and their families.
• Develop, implement, and adjust individualized treatment plans based on patient needs and progress.
• Prescribe medications, order diagnostic tests, and interpret results to provide holistic care.
• Educate patients and families on health management strategies, preventive care, and lifestyle modifications.
• Collaborate with physicians and other healthcare professionals to coordinate care and ensure continuity.

Qualifications:

• Master’s or Doctorate degree in Nursing (MSN or DNP) from an accredited program.
• Current and valid Nurse Practitioner license in Washingon with prescriptive authority.
• Minimum of 2 years of clinical experience, preferably in home healthcare, family practice, or geriatrics.
• Strong diagnostic skills, clinical judgment, and ability to work independently.
• Excellent interpersonal and communication skills, with a focus on compassionate patient care.
