Position Overview

Phoebe Hospice is seeking a compassionate and dedicated Nurse Practitioner to join our interdisciplinary team in Albany, Georgia. This role focuses on delivering high-quality, patient-centered care to individuals facing life-limiting illnesses across Southwest Georgia. The Nurse Practitioner will play a vital role in supporting patients and their families by enhancing quality of life through thoughtful, personalized care and clinical expertise.

Phoebe Hospice provides specialized care for patients with a limited life expectancy, emphasizing comfort, compassion, and dignity at every stage of the end-of-life journey. While hospice is known for emotional support, our program also delivers excellence in symptom control and pain management to enhance quality of life.

Key Responsibilities

  • Provide comprehensive medical care to patients in accordance with established protocols and standards
  • Conduct patient assessments, develop care plans, and collaborate with physicians and the interdisciplinary care team
  • Manage and treat symptoms, including pain and other distressing conditions
  • Support patients and families through education, communication, and compassionate presence
  • Participate in care coordination and case discussions to ensure continuity and quality of care
  • Maintain accurate and timely documentation in compliance with regulatory requirements

Qualifications

Master's degree in nursing. (Required)

Work Experience

3 years' experience in supervision and independent decision making in major nursing areas. (Preferred)

Licenses and Certifications

  • NP-C - Adult Nurse Practitioner - AANPCP (Required)
  • BCLS - Basic Life Support (Required)
