Nurse Practitioner Hospice

Join Gentiva as a Hospice Nurse Practitioner PRN and provide compassionate, home-based care to patients with life-limiting illnesses. You will conduct comprehensive assessments, manage pain and symptoms, and develop individualized care plans in collaboration with an interdisciplinary team. Educate and support families, ensure regulatory compliance, and document care in the EHR.
