Buckeye Hospice Care is growing in Cleveland and is looking for a Nurse Practitioner to join our team! The Nurse Practitioner (NP) is a registered professional nurse with advanced graduate education and clinical training who provides comprehensive hospice services in compliance with the state Nurse Practice Act. Working under the supervision of a collaborating physician, the NP delivers high-quality hospice care through clinical evaluations, symptom management, and ongoing coordination with the interdisciplinary team. Responsibilities : Clinical Care - Provide medical management, symptom control, and oversight of terminal illness under physician collaboration.
- Perform and document clinical assessments, including history/physical and required hospice Face-to-Face evaluations.
- Develop and adjust plans of care, prescribe treatments, and coordinate consults/referrals.
- Partner with the interdisciplinary team to ensure patient comfort, safety, and continuity of care.
- Communication & Coordination
- Collaborate with attending physicians, team members, and community providers to support eligibility decisions and care goals.
- Maintain accurate clinical documentation and participate in interdisciplinary team meetings.
- Educate patients and families on disease progression, hospice philosophy, and available support.
Compliance & Professional Standards - Adhere to Medicare/Medicaid regulations, HIPAA requirements, and hospice Conditions of Participation.
- Participate in performance improvement and maintain professional knowledge through ongoing education.
