Qualifications:
- Master of Science in Nursing (MSN) or higher from an accredited Nurse Practitioner program.
- Active New York State Nurse Practitioner license (required).
- Board Certified as a Nurse Practitioner.
- New graduates are welcome to apply.
- Bilingual in Russian.
- Proficiency in comprehensive patient assessments, diagnosis, treatment planning, and documentation; comfort with point-of-care testing and portable medical equipment.
- Highly independent, with excellent clinical judgment, strong communication skills, compassion, and adaptability to different home environments.
Key Responsibilities:
- Perform comprehensive health assessments and physical examinations in patients' homes.
- Diagnose and manage acute and chronic health conditions.
- Develop and implement individualized care plans.
- Prescribe medications and treatments within the NP scope of practice.
- Order and interpret diagnostic tests.
- Provide patient and family education on health maintenance and disease management.
- Coordinate care with primary care physicians and other healthcare providers.
- Maintain accurate and timely documentation of all patient encounters.
