Position Overview:

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team. The ideal candidate will have experience in primary care, be committed to providing patient-focused care, and be capable of working independently as well as part of a collaborative team. This role involves delivering comprehensive care to patients of all ages, managing chronic conditions, and promoting preventive health measures.

Key Responsibilities:

• Conduct thorough physical exams, assess patient health, and diagnose and manage acute and chronic conditions.
• Develop and implement individualized care plans in collaboration with patients, families, and the healthcare team.
• Prescribe medications, treatments, and interventions as appropriate.
• Provide patient education on health promotion, disease prevention, and management of chronic conditions.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care.
• Maintain accurate and up-to-date patient records using our electronic health record (EHR) system.
• Participate in quality improvement initiatives and contribute to the continuous improvement of practice operations.

Qualifications:

• Master's degree in Nursing (MSN) from an accredited program.
• Current Nurse Practitioner (NP) license in the state of Indiana.
• Board certification as a Family Nurse Practitioner (FNP).
• Minimum of 3 years of experience as a Nurse Practitioner.
• Strong clinical skills and ability to make sound decisions in patient care.
• Excellent communication, interpersonal, and organizational skills.
• Proficiency in using electronic health records (EHR) systems.
