We are seeking an experienced and compassionate Nurse Practitioner (NP) to join our team providing care in Skilled Nursing Facilities (SNFs). This full-time role offers stability, strong clinical autonomy, and the opportunity to provide meaningful care to patients in a post-acute and long-term care setting.
This position is ideal for a Nurse Practitioner who enjoys managing complex patient populations, working collaboratively with interdisciplinary teams, and practicing in a structured facility environment.

Responsibilities
- Perform comprehensive patient assessments in Skilled Nursing Facility settings
- Diagnose and manage acute and chronic medical conditions
- Conduct routine patient rounds and follow-up visits
- Collaborate with physicians, nursing staff, and interdisciplinary team members
- Order and interpret labs, diagnostic studies, and treatment plans
- Educate patients and families regarding care plans and treatment goals
- Complete accurate and timely documentation in the EMR
- Ensure compliance with Medicare and facility regulations
Qualifications
- Active Nurse Practitioner (NP) license
- FNP, AGPCNP-BC (Primary Care, ANCC), A-GNP (Primary Care, AANPCB), or AGACNP-BC (Acute Care, ANCC) APRN Any of these Boards Certifications.
- Experience in Skilled Nursing Facilities, post-acute care, or long-term care required
- Strong clinical decision-making and patient management skills
- Comfortable managing a patient panel independently
- Ability to work collaboratively with physicians and facility staff
