    Description

    Matrix Medical Network seeks a Nurse Practitioner to deliver high-quality primary and preventive care within our Healthcare & Medical Services team. You will perform in-home and hybrid assessments, manage chronic conditions, and develop evidence-based care plans that improve outcomes and reduce costs. In this collaborative, innovation-driven environment, you will work closely with an interdisciplinary team, leverage telehealth and EHR tools, and contribute to quality initiatives.
