(Contract to hire) Working between multiple LTC facilities in Nash, Wake, and Wayne Counties Schedule: Monday-Friday (8am-5pm) Contract: 3-6 month, contract to hire Pay Rate: $65-70/hour Specific Duties: Provide specialized care to high-risk health plan members Work as part of an interdisciplinary team Conduct comprehensive assessments Manage acute and chronic medical and behavioral conditions Ensure accurate documentation, including ICD-10 coding Communicate and collaborate with the care team Facilitate advanced illness and care planning discussions Offer counseling and education to patients and caregivers Perks: No on-call No weekends No holidays required Must have qualifications/experience: Nurse Practitioner or Physician Assistant NP Certified in one of the following (FNP, ANP, GNP) Access to transportation * 1+ year of experience (LTC, SNF, Geriatric preferred) Apply for this role with Pride Health!
