Position Overview
The Nurse Practitioner (NP) will provide high-quality, evidence-based primary and family care within a collaborative clinical care team. This role focuses on delivering comprehensive patient care, improving health outcomes, and supporting patients in managing acute and chronic conditions.
The NP will work closely with a multidisciplinary team while maintaining autonomy in clinical decision-making.

Key Responsibilities: 
  • Provide primary and family care, including assessment, diagnosis, treatment, and management of health conditions
  • Deliver patient-centered care, including health counseling and disposition planning
  • Conduct routine exams, diagnostic testing, and treatment for acute and chronic conditions
  • Promote preventive care and healthy lifestyle choices through screenings and wellness education
  • Manage chronic disease conditions and coordinate care plans with other healthcare professionals
  • Encourage patient engagement in their own health and well-being
  • Perform administrative duties including:
  • Clinic opening/closing procedures
  • Inventory management
  • Insurance verification and payment collection
  • Patient follow-ups and appointment management
  • Maintain accurate documentation and ensure a clean, compliant clinical environment

Required Qualifications: 
  • Master’s Degree from an accredited Family Nurse Practitioner (FNP) program
  • Current National Board Certification
  • Active, unrestricted State NP license
  • Minimum 1+ year of recent Primary Care, Family Medicine, or Urgent Care experience as an NP
  • Strong knowledge of chronic disease management and quality measures
  • Excellent communication and patient counseling skills
  • Ability to work independently and within a team-based care model
  • Proficiency with EMR systems and healthcare technology

Preferred Qualifications: 
  • Experience in value-based care models
  • Willingness to obtain multi-state licensure
  • Strong organizational, multitasking, and problem-solving skills
