Position Summary
The Nurse Practitioner provides direct primary care services to patients across the lifespan, including diagnosis, treatment planning, and coordination of care. Works collaboratively with physicians and the care team to deliver high-quality, patient-centered care, and serves as a key clinical resource within Wilmington Community Clinic's primary care team.


Benefits
The compensation for this position is $130,000.00 - $145,000.00 annually. Education and Experience

  • Master's or Doctoral degree in Nursing (MSN/DNP) from an accredited college or university, with certification as a Nurse Practitioner.
  • Valid Nurse Practitioner license and Furnishing License issued by the California Board of Registered Nursing.
  • Active National Provider Identifier (NPI), current BLS/CPR certification, and DEA registration as applicable to role.

Essential Position Responsibilities

  • Provides comprehensive primary care services, including history-taking, physical examination, diagnosis, and treatment planning for patients across the lifespan.
  • Orders, performs, and interprets diagnostic tests; prescribes medications and treatments within scope of practice and California furnishing authority.
  • Develops and implements individualized care plans for patients with acute and chronic health conditions.
  • Provides preventive care, health education, and counseling to patients, families, and caregivers.
  • Assists patients in navigating their care by connecting them with appropriate WCC and community resources as needed.
  • Assists with administration, development, implementation, and evaluation of clinical programs and protocols.
  • Collaborates with physicians in reviewing and co-signing charts, as required under California furnishing regulations.
  • Knowledge of computer operations;  ability to readily adapt to the EPIC electronic health record system.
  • Follows clinic policies and procedures to ensure that the principles of WCC are implemented.
  • Fosters an environment that promotes trust and cooperation among all staff of WCC.
  • Attends all WCC mandatory meetings and other meetings as requested.
  • Interacts with patients, physicians, staff, vendors, and visitors in ways that demonstrate caring and reflect the WCC mission and philosophy.
  • Completes other duties as assigned in support of WCC's mission.

  • Practices in a professional manner, and collaborates with other healthcare team members.
  • Works successfully with a variety of individuals from various socio-economic, ethnic and cultural backgrounds.
  • Maintain computer skills adequate to perform EPIC duties within 90 days of employment.
  • Knowledgeable of clinical guidelines, evidence-based practice standards, and primary care procedures and resources.
  • Ability to communicate effectively.
  • Bilingual English/Spanish preferred.
  • Punctuality and excellent attendance required.
