The Department provides care to a population of Veterans, Veteran Spouses and Gold Star Parents. and is seeking: no more than two (2) Nurse Practitioners, or Physician Assistants. These positions are to provide services onsite at least five (5) days a week between the hours of 8:00 AM and 4:30 PM, Monday through Friday. They must also provide on-call medical coverage at all times (24 hours, 7 days a week) in the event there is no provider in the facility.
Physician support staff services shall include, but are not limited to, routine and as-needed resident evaluations, coordination of care, documentation in the facility's electronic medical record system, and participation in care planning processes. The vendor must ensure adequate staffing levels to meet the clinical demands of the Department, maintain continuity of care, and support quality outcomes.
May also Perform physical examinations, diagnose illnesses and conditions and prescribe a plan of corrective care, exclusively within their specialty area of practice in collaboration with a licensed physician. Order, interpret and evaluate diagnostic tests to identify and assess patient's clinical problems and health care needs. Record physical findings, and formulates plan and prognosis, based on patient's condition. Discuss case with physician and other health professionals to prepare comprehensive patient care plan. Submit health care plan and goals of individual patients for periodic review and evaluation by physician. Prescribe or recommend drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures. May refer patients to physician for consultation or to specialized health resources for treatment. May be designated according to field of specialization.
Must have a New York State License to practice as a Registered Professional Nurse; and certification by the New York State Education Department in a specialty area to practice as a Nurse Practitioner.

Salary commensurate with scope of work job experience
Proper footwear required to perform tasks in a safe manner.
Perform other reasonably related duties as required.
