About the Nurse Practitioner Position:

Key Responsibilities:

• Provide care to Subacute Rehab and Long-Term Care residents on our Skilled Nursing Unit and our Personal Care Unit.
• Collaborate with other NPs, Therapists, Nurses and Physicians to provide quality care.
• Examine residents and develop a treatment plan in coordination with other disciplines.
• Order and interpret diagnostic tests.
• Prescribe appropriate medications and therapies.
• Ensure proper documentation in our Electronic Medical Record.
• Ensure that residents and their families are kept apprised of clinical progress during their stay.

Qualifications:

• Current and unrestricted Pennsylvania State NP License.
• Current and unrestricted DEA License.
• Desire to work in a subacute rehabilitation, long-term care, and personal care environment.
• Outstanding written and verbal communication skills.
• Ability to document decisions, risks, and treatment plans clearly and effectively.
• Proficiency in using electronic medical records systems.
• Ability to work independently and collaboratively in our healthcare setting.
• Commitment to maintaining up-to-date knowledge through continuing education and professional development.
