Summit Healthcare Staffing is hiring on behalf of a client who is seeking an experienced Nurse Practitioner to join their expanding team in Brooklyn, NY. This is an in-person position where you'll provide care through a telehealth platform in a modern, collaborative clinical environment.

Responsibilities

Conduct patient evaluations and follow-up visits through a telemedicine platform
\n
Diagnose, treat, and manage a variety of acute and chronic conditions
\n
Develop and implement individualized treatment plans
\n
Maintain accurate and timely electronic medical records
\n
Collaborate with physicians and clinical staff to ensure exceptional patient care
\n
See approximately 25 patients per day
\n
\n
Qualifications
\n
\n
Active Nurse Practitioner license in New York
\n
Active Medicaid and Medicare ID numbers required
\n
Strong clinical assessment and communication skills
\n
Telehealth experience is a plus
\n
Ability to thrive in a fast-paced, patient-focused environment
\n
