The position will cover Manhattan with coverage for Brooklyn when needed.

Responsibilities:

Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPNs who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.

 

In collaboration with physician, the NP will be providing comprehensive assessment, including multifactorial complex
physical examination and psychosocial needs, self - management education & tools to better manage patient’s multiple conditions. As a part of Interdisciplinary Co-Management Program, the NP will be providing and coordinating primary care services to an assigned the at risk panel of Elderplan members who reside in the community. The NP will insure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives. The NP will act as clinical resource to the member, family and interdisciplinary team.

  • You will insure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives
  • Provide and/or coordinate primary clinical care services for assigned Nursing Home Residents
  • You will act as a clinical resource to the participating providers and facilities
  • You will act as a resource and contact for member/designated representative regarding all aspects of member individualized care plan and disease process
Qualifications:
  • Masters Degree in Nursing, graduate of accredited Nurse Practitioner program required
  • One year of clinical experience in role of nurse practitioner required
  • Minimum one year in geriatric specialty
  • Clinical RN/NP experience in acute, subacute and/or long term care setting preferred
  • Prior experience in manage care a plus
  • Ability to assess, diagnose, treat and follow-up
  • Ability to work independently and as part of interdisciplinary teams
  • Ability to develop relationships and communicate to members/family
  • Establish priorities and maintain schedules
  • NYS NP License, Registration, DEA certificates
  • Car and valid driver license required
Min: USD $140,000.00/Yr. Max: USD $150,000.00/Yr.
