As a member of the interdisciplinary team, theNurse Practitioner will provide meaningful, end-of-life care where it matters mostat home, in nursing facilities, and at our Kaplan Family Hospice Residence. Youll be joining a dynamic group of NPs and physicians in a collaborative environment where your voice matters. The work of a hospice Nurse Practitioner is impactful and gratifying. This is the kind of medicine youve always dreamed of practicing. Practice with purpose. Work with heart. Schedule: M-F, 8:30am to 4:30pm. One (1) weekend on call per month. One (1) to two (2) holidays on-call per year. Responsibilities may include: Conducting in-home patient visits for symptom management, collaboration of care, goals of care discussion, assessment of complex wounds, and evaluation of eligibility for hospice benefits.In collaboration with the medical director and intake nursing staff, reviewing clinical documents to determine eligibility and terminal diagnosis. Serving as the agencys on-call provider, which includes advising nursing staff on symptom management, prescribing medications including controlled substances, and completing death certificates.Serving as a clinical expert to nursing and other staff.Participating in community education and outreach.
