    Compensation:$120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

    As a full-time Wound Care Nurse Practitioner, you'll serve senior living communities throughout the Twin Cities Metro, providing advanced wound care and clinical support directly where patients live. You'll work with residents facing complex medical challenges while helping them maintain dignity, comfort, and quality of life.

    What You'll Be Doing

    • Provide advanced wound care assessments, treatment planning, and ongoing management of complex wounds.

    • Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities.

    • Manage medications, diagnostics, treatment plans, and follow-up care.

    • Collaborate with physicians, nurses, therapists, caregivers, and community staff.

    • Support proactive interventions that improve healing outcomes and reduce avoidable complications.

    • Help patients maintain the highest possible quality of life throughout their healthcare journey.

    Qualifications

    • Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred).

    • Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred.

    • Strong clinical judgment and confidence managing medically complex patient populations.

    • Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
