A PACE (Program of All-Inclusive Care for the Elderly) center opening in the Stockton, CA area is seeking a Nurse Practitioner to join its interdisciplinary care team. PACE is a capitated care model funded through Medicare and Medicaid that supports frail, elderly participants living safely in their homes and communities through comprehensive, coordinated care delivered in a center-based, outpatient clinic setting. This is not a home visit or home care position; participants are seen at the PACE center.
Responsibilities:
Provide primary care services to a panel of PACE participants, most of whom are frail, elderly individuals with complex, multiple chronic conditions, in collaboration with the center's supervising physician. Conduct comprehensive health assessments, diagnose and manage acute and chronic conditions within scope of practice, and develop individualized treatment plans. Serve as an active member of the interdisciplinary care team (IDT), collaborating with nursing, social work, rehabilitation therapy, dietary, recreation, and personal care staff. Participate in regular IDT meetings. Coordinate participant care across settings, including center-based visits, home care coordination, and specialist or hospital referrals. Order and interpret diagnostic testing, and prescribe and manage medications within scope of practice and collaborative agreement. Maintain accurate, timely documentation in the electronic medical record. Support quality improvement initiatives and compliance with CMS and state PACE program requirements. Participate in an on-call rotation as needed.
Requirements:
Master's or Doctoral degree in Nursing from an accredited Nurse Practitioner program. Active, unrestricted California Nurse Practitioner license, or ability to obtain prior to start. National certification (ANCC or AANP) in Family, Adult-Gerontology, or Geriatric practice preferred. Current DEA registration, or ability to obtain. Experience with geriatric or medically complex populations preferred. Prior experience in a PACE program or similar interdisciplinary care model preferred.
