A healthcare organization is seeking a Nurse Practitioner (NP) in New York City to support Clinical Health Assessments through a hybrid mix of telehealth and in-person care. This role focuses on delivering patient-centered assessments in a flexible clinical setting.
About the Opportunity:
- Schedule: Hybrid (varies by assignment)
- Hours: Minimum 15 hours per week
- Setting: Hybrid
Responsibilities:
- Conduct Clinical Health Assessments in both telehealth and face-to-face settings
- Perform patient evaluations and document clinical findings accurately
- Support assessment workflows and maintain quality standards for care delivery
- Communicate effectively with patients throughout the assessment process
- Complete required training and apply learned procedures in daily practice
Qualifications:
- Active Nurse Practitioner (NP) credentia
- lMinimum availability of 15 hours per week
- Completion of 40 hours of training in one continuous week
- Experience providing clinical assessments or patient evaluations
Desired Skills:
- Experience with telehealth and in-person care delivery
- Familiarity with Clinical Health Assessments
- Strong documentation and patient communication skills
