    Compensation:$120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

    What You'll Be Doing

    • Provide advanced wound care assessments, treatment planning, and ongoing management.

    • Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities.

    • Manage pressure injuries, diabetic wounds, surgical wounds, skin integrity concerns, and related conditions.

    • Coordinate care with physicians, nursing staff, therapists, caregivers, and community leadership.

    • Support chronic disease management, preventative care initiatives, and continuity of care.

    • Help improve clinical outcomes while reducing avoidable complications and hospitalizations.

    Qualifications

    • Active Minnesota Certified Nurse Practitioner (CNP) license.

    • Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing strongly preferred.

    • Wound care certification is highly valued.

    • Strong clinical judgment, professionalism, and confidence managing medically complex patient populations.

    • Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
