Nurse Practitioner / Physician Assistant – Geriatric Care Focus

This role centers on building relationships, coordinating care across disciplines, and managing chronic conditions in a way that keeps patients healthier and at home.

About the Role

• Deliver primary and preventive care to older adults within a structured care model
• Conduct assessments, exams, and routine medical procedures
• Order and interpret lab work, manage medications, and respond to urgent needs
• Collaborate with physicians, nurses, and social workers to develop care plans
• Provide education and support to participants and families
• Participate in care planning, quality improvement efforts, and after-hours coordination as needed

What You'll Need

• Active certification and licensure as a Nurse Practitioner or Physician Assistant
• Experience working with geriatric or medically complex populations preferred
• CPR and First Aid certification
• Ability to work effectively in a team setting and build rapport with patients
• Comfortable managing care across home, clinic, or community-based settings
• Valid driver’s license and ability to travel locally when needed
