Nurse Practitioner (NP) – Home Visits | Bilingual Russian or Chinese

Position Overview

A large multispecialty group in New York City is actively seeking a Nurse Practitioner (NP) to deliver high-quality, patient-centered care in patients’ homes and nursing facilities. This opportunity is well-suited for providers who value holistic care, chronic disease management, and meaningful patient interaction.

Key Responsibilities

Patient Care & Evaluation

• Perform comprehensive patient assessments, including physical exams, medication reconciliation, and detailed medical history reviews during home and facility visits
• Diagnose acute and chronic conditions and develop individualized treatment plans
• Prescribe medications and manage ongoing chronic illnesses

Care Coordination & Collaboration

• Work closely with physicians, specialists, and interdisciplinary teams to ensure continuity and quality of care
• Coordinate referrals and follow-ups with appropriate healthcare providers

Patient & Family Education

• Educate patients and families on disease management, lifestyle changes, medication adherence, and preventive care strategies

Documentation & Follow-Up

• Maintain accurate and timely documentation within EMR/EHR systems
• Conduct follow-up visits to monitor patient progress and adjust care plans as needed

Emergency Response & Community Engagement

• Assess and manage urgent medical concerns during home or facility visits
• Participate in community outreach and patient education initiatives

Qualifications

• Master’s Degree from an accredited Nurse Practitioner program (Family NP or Adult/Geriatrics preferred)
• Active New York State NP license
• National Board Certification (AANP, ANCC, or ONCC)
• Bilingual proficiency in Russian or Chinese preferred due to patient population
• Experience with EMR/EHR systems preferred
• Strong clinical judgment, communication skills, and ability to work independently
