Overview

Tenavi Staffing is partnering with a rapidly growing healthcare organization focused on improving outcomes for Medicare Advantage members with complex and chronic health conditions. We are seeking a Nurse Practitioner or Physician Assistant to provide in-home assessments and care coordination support throughout Wake County, North Carolina.

This is an excellent opportunity for a clinician who enjoys building meaningful patient relationships, working independently in the field, and improving the quality of care for seniors in their homes.

Position Overview

The Advanced Practice Clinician will conduct comprehensive in-home health assessments for Medicare-age patients, helping identify care gaps, chronic condition management needs, and opportunities to improve overall health outcomes. Visits typically range from 45 to 60 minutes and focus on delivering personalized, patient-centered care.

Key Responsibilities

• Conduct in-home patient assessments throughout Wake County
• Perform comprehensive history and physical examinations
• Complete medication reviews and reconciliation
• Conduct cognitive, depression, and safety screenings
• Identify chronic disease management needs and care gaps
• Communicate assessment findings and recommendations to primary care providers
• Educate patients, caregivers, and families on treatment plans and health management
• Facilitate referrals to community resources and support services
• Participate in end-of-life and advanced care planning discussions when appropriate
• Collect laboratory specimens in the home setting as needed
• Maintain accurate documentation and EMR records
• Ensure HIPAA compliance and patient confidentiality

Required Qualifications

• Master's Degree from an accredited Nurse Practitioner or Physician Assistant program
• Active North Carolina Nurse Practitioner or Physician Assistant license
• Active Board Certification as an NP or PA
• NPI Number
• DEA Registration
• Current BLS Certification
• Valid North Carolina driver's license
• Current automobile insurance
• Ability to travel throughout Wake County for home visits
• Strong communication, organizational, and time management skills

Preferred Qualifications

• One or more years of clinical experience
• Home health, primary care, geriatrics, hospice, palliative care, or care management experience
• Experience working with Medicare Advantage populations
• EMR documentation experience
• Experience caring for older adult or geriatric patients

 

Compensation & Benefits

• Salary Range: $109,932 - $164,000 DOE
• The pay range may be based on several factors, including market location, education, responsibilities, and experience.
