Salary: Starting at $130,000/year

Schedule: 4 day workweek-Monday, Wednesday, Friday and 4th day flexible

What You'll Do

In collaboration with our Medical Director and Behavioral Health leadership, you'll be on the front lines of expanding access to life-changing care. Your role will include:

  • Conduct evaluation with relevant history, health assessment based on the patient's signs & symptoms
  • Identify needs of the patient and make a differential diagnosis based on the clinical presentation, history and clinical findings.
  • Conduct waived testing for urine, blood work and glucose monitoring
  • Develop a prioritized and mutually acceptable plan of care according to the written practice protocols in collaboration with the physician.
  • Implement a treatment plan that is prioritized and mutually acceptable with appropriate diagnostic tests, pharmacological and non-pharmacological interventions, patient education, and with appropriate referrals as needed.
  • Provide systematic follow-up by evaluating the effectiveness of various treatments provided, therapeutic processes used, their outcome and modifying treatments & management plans as necessary to achieve the optimum mental health goals.
  • Provide educational materials & information needed to make well informed choices and decisions by patients about their health promotion, illness prevention and finally, illness treatment.
  • Provide a safe environment for the patient, and promote patient advocacy at the individual, community, and global levels.
  • Participate with other professional colleagues to provide comprehensive health care.
  • Maintain accurate, legible and confidential medical records.
  • Responsible for conducting medical groups and educational seminars for patients.

What You Bring

  • NYS Licensed NP + NYSED certification

  • Master's degree
  • Current/valid New York State Registered Nurse and Nurse Practitioner (NP) license
  • Currently registered as a registered professional nurse and NP certification in good standing
  • Certified as a nurse practitioner pursuant to 6910, with prescription writing privileges pursuant to §6902.3b of the Education Law
  • DEA license
  • Hold all requirements indicated by the NY Department of Drug and Alcohol Program to perform history and physicals and prescribe medications
  • Current basic life support for health providers (BLS)
  • Five (5) years of related work experience
  • ANCC credentialing and certification
  • Must demonstrate competence in the specific areas relevant to this role, as assessed during competency reviews (see role competency).
  • Bilingual (Spanish/English OR Russian/English) is a big plus!
