Position Description: We are looking for a dedicated and compassionate Nurse Practitioner ( NP ) for our Skilled Nursing Facility ( SNF ) in Old Bridge, NJ for bedside patient care. The NP will serve as the primary medical provider for assigned residents, working closely with the Medical Director, facility staff, and interdisciplinary teams. This is a full-time, in-person position Monday through Friday 9am to 5pm. No weekends, no nights, no on-call.

Qualifications of the Nurse Practitioner ( NP ):


  • Valid Nurse Practitioner ( NP ) license in the state of New Jersey, in good standing

  • Active DEA registration

  • 1+ years of experience as a Nurse Practitioner; SNF, geriatric, or post-acute experience preferred

  • Strong clinical documentation skills and familiarity with EMR systems

  • Excellent communication skills with patients, families, and interdisciplinary teams

Responsibilities of the Nurse Practitioner ( NP ):


  • Upon admission, examine residents and review their medical records to establish baseline assessments

  • Conduct routine follow-up visits and manage chronic conditions including diabetes, hypertension, CHF, COPD, and dementia

  • Prescribe and adjust medications including controlled substances per state regulations

  • Lead transitional care management and discharge planning with social work, nursing, and rehabilitation teams

  • Engage with residents' families on care plans, goals of care, and advance directives
