• Outpatient Primary Care Only No inpatient or on-call responsibilities

• Patient Volume: 15 17 patients/day

Responsibilities

• Provide comprehensive primary care to a senior patient population

• Manage chronic conditions and preventive care

• Address patients' medical and social needs

• Collaborate with physicians, RNs, and other care team members

• Perform basic in-office procedures as appropriate

Qualifications

• NP with 2+ years of outpatient primary care experience

• Active state license

• Strong patient-centered approach
