Community Health Partners (CHP) is seeking a full‑time Nurse Practitioner (NP) or Physician Assistant (PA) to join our established outpatient Medical Oncology practice in Fresno, CA. This is an exceptional opportunity to work alongside a collaborative team of Medical Oncologists, Hematologists, and experienced Advanced Practice Providers in a comprehensive cancer center dedicated to delivering high‑quality, patient‑centered oncology care.

Responsibilities

  • Provides comprehensive oncology assessment, evaluation, and direct patient care in accordance with established protocols.
  • Manages patients collaboratively with supervising physicians, including participation in daily rounds, treatment planning, and follow‑up care.
  • Orders, interprets, and evaluates diagnostic tests such as CBC, CMP, tumor markers, imaging studies, and pathology reports.
  • Furnishes medications—including chemotherapy‑related supportive agents—based on standardized procedures.
  • Educates patients and families on diagnosis, treatment options, chemotherapy/immunotherapy regimens, side‑effect management, and survivorship planning.
  • Coordinates referrals to radiation oncology, surgical oncology, and other subspecialties as needed.
  • Obtains prior authorizations for oncology medications, imaging, and procedures.
  • Documents patient encounters thoroughly and accurately in the electronic medical record.
  • Identifies and differentiates between normal and treatment‑related abnormal findings, escalating concerns appropriately.
  • Participates in quality improvement initiatives, policy development, and evidence‑based practice updates.
  • Performs other duties as assigned to support high‑quality oncology care.
  • Qualifications

    Education

    • Completion of an accredited Nurse Practitioner or Physician Assistant program required
    • Credentialing by the CMC Medical Staff Office as an Allied Health Professional with privileges for oncology‑related standardized procedures required

    Experience

    • Minimum 3 years of clinical experience required
    • 1 year of oncology or hematology experience preferred

    Licenses & Certifications

    If qualifying as a Nurse Practitioner:

    • Current California NP License
    • Nurse Practitioner Furnishing Number (NPF)
    • National certification (ANCC or AANP) preferred

    If qualifying as a Physician Assistant:

    • Current California PA License
    • DEA Controlled Substance Registration
    • Ability to participate in federally funded programs (Medicare/Medicaid)

    For all candidates:

    • Current American Heart Association Basic Life Support (BLS) certification
