Nurse Practitioner or Physician Assistant The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community. Primary Purpose In this role, you'll provide compassionate, high-quality primary care to Senior Care Partners PACE participants in collaboration with a licensed physician, supporting their health, independence, and well-being. Key Responsibilities - Performs comprehensive history and physicals for new referrals.
- Performs interval history and physicals on established participants, completed quarterly or as indicated.
- Evaluates and treats participants with episodic illnesses.
- Assists the IDT in formulating a plan for the coordination of 24-hour care.
Leadership Responsibilities This position has no professional leadership responsibilities. Qualifications - Bachelor’s degree required, Master’s degree preferred.
- Must be a graduate of an accredited Nurse Practitioner educational program.
- Must be licensed as a Registered Nurse in the State of Michigan.
- Must be licensed as a Nurse Practitioner in the State of Michigan.
- Minimum of 3 years of experience as a PA/NP
- Minimum of one year of experience with frail elderly population.
- Minimum of one year home care experience preferred.
- Minimum of one-year wound care experience preferred.
Specific Requirements - Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by Senior Care Partners PACE and approved by CMS before working independently.
- Must be knowledgeable of current medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
- Must be willing to seek out new methods and principles and be willing to incorporate them into existing medical practices.
Work Environment - Has a visible presence while working in the center and is subject to frequent interruptions.
- Has a working presence in the community and is involved with participants, family members, personnel, visitors, government agencies/personnel, etc., under all conditions/circumstances.
- Works beyond normal working hours and on weekends and holidays when necessary.
- Participates in an on-call rotation
- Subject to callback during emergency conditions (e.g., severe weather, evacuation, disaster, etc.).
Minimum Working Requirements - Must have reliable transportation, current valid Michigan driver’s license and car insurance.
- Must be 18 years of age or older.
- Must be fully vaccinated, or willing to be fully vaccinated against the following communicable diseases: Influenza, MMR, Varicella, Tdap/Td, Covid-19 and Hepatitis B.
