Nurse Practitioner or Physician Assistant — Skilled Nursing FacilityAbout the Role

We're looking for an experienced, autonomous Nurse Practitioner or Physician Assistant to join our clinical team full time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity — you'll manage your own panel and build long-term relationships with patients and their families.

Your Day-to-Day

You'll see roughly 16–20 patients a day — a mix of long-term chronic care and acute issues. That means annual wellness visits and H&Ps, managing chronic and co-morbid conditions, responding to acute changes, reviewing meds and orders, and coordinating with collaborating physicians, specialists, and facility staff to keep care seamless. You'll also stay in touch with families and join care conferences where it makes sense.

Qualifications

  • Board certification required (e.g., AGNP, FNP for Nurse Practitioners; PA-C for Physician Assistants) — active and unrestricted.
  • Valid, unrestricted Nurse Practitioner or Physician Assistant license in the state of operation.
  • Master's degree in Nursing (Nurse Practitioner) or from an accredited Physician Assistant program — required for the corresponding credential.
  • Skilled nursing or long-term care experience strongly preferred.
  • Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.
