Job Summary:

The Geisinger Skilled Nursing Program is seeking an experienced Nurse Practitioner or Physician Assistant at Mountain View Rehabilitation and Senior Living Center in Coal Township, PA.

Job Duties:

This position is for a Geisinger provider working within a contracted skilled nursing facility. It requires an experienced advanced practitioner with prior clinical experience in skilled nursing, family medicine, internal medicine, or intensive care.The advanced practitioner (AP) provides direct (hands-on) care in skilled nursing facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting.

Responsibilities:

• Review reports
• Communicate with SNF staff for patient needs
• Treat patients for acute needs, admissions, discharges, etc.
• Manage extremely complex patients, see every day or at least once a week
• Approximately 12 patients a day
• Length of time per patient and number of patients is per need
