Nurse Practitioner for our Skilled Nursing Facility (SNF)

Compensation: $110,000 - $140,000 Annual Salary Benefits: Medical, dental, vision, life insurance, long-term and short-term disability insurance, malpractice insurance, 401K with match, PTO, and CME reimbursement.

Position Description

We are looking for a dedicated Nurse Practitioner to serve as the primary care provider for our skilled nursing facility in Wayne, MI. This is a full-time, in-person position, Monday through Friday, 9:00 AM to 5:00 PM, with no on-call and no weekends. The last two hours of the day can be completed remotely. You will manage a panel of long-term care and short-stay rehab residents, handling chronic disease management, acute changes in condition, and coordination with the facility's nursing and therapy teams.

Qualifications of the Nurse Practitioner:

Active and unrestricted Michigan Nurse Practitioner license in good standing

Active DEA registration

Board certification as an FNP, AGNP, or AGPCNP

Experience in skilled nursing, long-term care, or geriatrics preferred

Responsibilities of the Nurse Practitioner:

Provide primary care to residents, including physical exams, assessments, and medication management

Diagnose and treat chronic and acute conditions within the NP scope of practice

Order and interpret labs and diagnostic tests, and adjust care plans accordingly

Collaborate with facility nursing staff, therapy, and the interdisciplinary team

Maintain accurate and compliant clinical documentation
