Job Summary:
Tryfacta is seeking a Nurse Practitioner for our client in Oxford, NY 13830. This is a 2-year contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!

Position Title: Nurse Practitioner
Location: Oxford, NY 13830
Duration: 2 years
Work Schedule: 8:00 AM and 4:30 PM

Responsibilities for this position include, but are not limited to:
  • Conduct routine and as-needed resident assessments, examinations, and follow-up care.
  • Develop, monitor, and adjust treatment plans in coordination with the interdisciplinary care team.
  • Document all resident encounters, orders, and clinical findings in the electronic medical record (EMR) system.
  • Participate in care planning meetings and collaborate with nursing leadership, physicians, and other healthcare professionals.
  • Provide onsite and on-call clinical support while ensuring continuity of care and timely medical response.
To be considered for this position, you should have: [Skills, Education, or Experience]
  • Active New York State Nurse Practitioner (NP) license or Physician Assistant (PA) license with current certification.
  • Minimum experience in long-term care, skilled nursing facilities, geriatrics, rehabilitation, or related healthcare settings.
  • Strong clinical assessment, diagnostic, treatment planning, and patient management skills.
  • Proficiency in electronic medical records (EMR) documentation and interdisciplinary care coordination.
  • Excellent communication, teamwork, and ability to provide responsive onsite and 24/7 on-call coverage.
