Nurse Practitioner - Palliative Care

Hawai'i Pacific Health is seeking a motivated and driven Nurse Practitioner to join our Palliative Medicine team on the island of O'ahu

What You'll Do:

• Provide compassionate, patient-centered palliative care focused on relief of symptoms, pain management, and support for patients and families facing serious illness.
• Develop individualized care plans by assessing and managing complex medical conditions, addressing physical, emotional, and psychosocial needs, and facilitating advance care planning.
• Collaborate in interdisciplinary team-based care and participate in quality improvement initiatives to enhance palliative care delivery and patient experience.

Qualifications:

• Hawaii Registered Nurse License

• Hawaii Advanced Practice Registered Nurse License (APRN)

• Current American Heart Association Health Care Provider Basic Life Support (BLS)

About Pali Momi Medical Center

With 118 beds and more than 500 physicians on its medical staff, Pali Momi Medical Center offers a full range of services for the communities of Central and West Oahu. The medical center has delivered many medical firsts for the community, including Central and West O'ahu's only interventional cardiac catheterization unit to detect and treat heart disease, and one of the largest comprehensive centers for cancer care. Pali Momi also has a fully integrated minimally invasive surgical suite, CT scan and MRI services, a women's center and the state's first retina center.
