Are you a dynamic Nurse Practitioner or Physician Assistant with a passion for patient care and a drive to make a difference? We are seeking an exceptional individual to join our team of dedicated healthcare professionals. This full-time, permanent position offers the opportunity to work in a collaborative, patient-focused environment where you will use your extensive experience in preventive care, care coordination, medication management/prescribing, and population health to provide comprehensive care to our diverse patient population.
Responsibilities:
As a Nurse Practitioner or Physician Assistant, you will play a critical role in our healthcare team. Your responsibilities will include:
1. Providing high-quality, patient-centered preventive care services, including routine check-ups, health-risk assessments, immunization, and screening tests.
2. Coordinating care for patients, organizing and managing their healthcare needs across multiple settings, and ensuring seamless transitions of care.
3. Prescribing and managing medications, monitoring their effectiveness, and making adjustments as necessary.
4. Implementing strategies to improve population health, including disease management programs, health promotion, and disease prevention activities.
5. Collaborating with other healthcare professionals to plan, implement, and evaluate patient care.
6. Educating patients and their families on disease prevention and healthy lifestyle habits.
7. Conducting research to improve patient care and outcomes.
Qualifications:
To be considered for this position, you must possess the following qualifications:
1. Current, unrestricted license as a Nurse Practitioner or Physician Assistant.
2. Minimum of 3 years of experience as a Nurse Practitioner or Physician Assistant.
3. Demonstrated expertise in preventive care, care coordination, medication management/prescribing, and population health.
4. Excellent communication and interpersonal skills, with the ability to engage patients and their families, and work effectively as part of a team.
5. Strong analytical and problem-solving skills, with the ability to make sound clinical decisions.
6. Commitment to continuous learning and professional development.
7. Ability to handle a high level of responsibility and work under pressure.
8. Strong organizational skills, with the ability to manage multiple tasks and priorities.
9. Experience with electronic health records (EHR) systems.
