    We are seeking an experienced and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to provide primary care services to patients in their homes and Assisted Living Facilities (ALFs).

    The provider will deliver comprehensive medical care to patients who may have difficulty accessing traditional office-based care. This position is ideal for an experienced NP or PA who is comfortable working independently, managing chronic medical conditions, evaluating acute concerns, and coordinating care with other healthcare providers.

    Responsibilities

    • Perform comprehensive medical evaluations and follow-up visits in patients' homes and ALFs.
    • Obtain and document complete medical histories and physical examinations.
    • Diagnose and treat acute and chronic medical conditions.
    • Manage chronic diseases such as hypertension, diabetes, COPD, CHF, hyperlipidemia, and other common medical conditions.
    • Review medications and make appropriate medication adjustments.
    • Order and interpret laboratory tests, diagnostic studies, and other appropriate testing.
    • Prescribe medications in accordance with applicable laws and regulations.
    • Develop individualized treatment plans for patients.
    • Identify changes in patient condition and determine when additional evaluation or hospitalization is necessary.
    • Coordinate care with primary care providers, specialists, home health agencies, hospice, caregivers, and facility staff as appropriate.
    • Communicate effectively with family members and caregivers regarding the patient's treatment plan and medical needs.
    • Provide appropriate preventive care and health maintenance.
    • Complete accurate and timely medical documentation for each visit.
    • Maintain appropriate communication with the practice regarding patient follow-up and significant clinical issues.
    • Provide care in accordance with established clinical standards, practice policies, and applicable state and federal regulations.

    Qualifications

    • Current and unrestricted Nurse Practitioner or Physician Assistant license in the state of practice.
    • Current DEA registration, if applicable.
    • Current state prescribing privileges.
    • Board certification as an NP or PA.
    • Current BLS certification.
    • Malpractice coverage appropriate for independent contract work.
    • Previous primary care, internal medicine, geriatrics, home health, ALF, or similar experience preferred.
    • Experience managing medically complex and elderly patients preferred.
    • Ability to work independently and make sound clinical decisions.
    • Strong assessment, communication, organizational, and documentation skills.
    • Reliable transportation and willingness to travel to patient homes and ALFs.

    Ideal Candidate

    The ideal candidate is an experienced NP or PA who is:

    • Experienced in managing elderly and medically complex patients.
    • Professional, dependable, and compassionate.
    • Comfortable communicating with patients, families, caregivers, and facility staff.
    • Able to efficiently complete visits and documentation.
    • Comfortable traveling between multiple patient locations.
    • Able to recognize when a patient's condition requires a higher level of care.

    Compensation

    Contract compensation will be based on experience, qualifications, and the agreed-upon visit structure.

    Compensation may be structured as a per-visit rate, hourly rate, or other mutually agreed-upon contract arrangement.

    Contractor Status

    This position is offered as an independent contractor position. The provider will be responsible for maintaining all required professional licenses, certifications, insurance, and credentials necessary to provide services.
