      Palliative Medicine Nurse Practitioner (NP) / Physician Assistant (PA)

      Full-Time | Monday-Friday | Day Shift

As a Palliative Medicine Nurse Practitioner or Physician Assistant, you will provide advanced clinical care to patients across both inpatient and outpatient settings, helping manage complex symptoms, support patient and family goals of care, and enhance the overall healthcare experience.

      This role offers a unique opportunity to combine clinical expertise, care coordination, patient advocacy, and education while working closely with physicians, nurses, social workers, chaplains, and other healthcare professionals to deliver comprehensive, patient-centered palliative care.

      Position Highlights

      • Full-Time

      • Monday-Friday, Normal Business Hours

      • 80% Inpatient Consult Service

      • 20% Outpatient Clinic

      • No nights, weekends, or call requirements (if applicable)

      What You'll Do

      Provide Advanced Clinical Care

      • Conduct comprehensive assessments of patients with acute, chronic, and complex medical conditions.

      • Evaluate physical, emotional, psychosocial, and symptom management needs.

      • Diagnose, treat, and manage health conditions within scope of practice and approved clinical privileges.

      • Perform therapeutic interventions and clinical procedures as appropriate.

      Develop & Manage Plans of Care

      • Collaborate with physicians and interdisciplinary team members to develop individualized treatment plans.

      • Monitor patient progress and modify care plans based on clinical response and changing needs.

      • Promote symptom management, comfort, quality of life, and patient-centered goals of care.

      • Facilitate smooth transitions across care settings, including inpatient units, outpatient clinics, and community resources.

      Support Patients & Families

      • Provide education and guidance regarding diagnoses, treatment options, symptom management, and care planning.

      • Assist patients and families in navigating complex healthcare decisions.

      • Serve as an advocate for patient preferences, values, and goals.

      Educate & Collaborate

      • Serve as a clinical resource for nursing staff, providers, patients, families, and interdisciplinary team members.

      • Participate in patient, family, and staff education initiatives.

      • Collaborate with hospital and clinic teams to promote high-quality palliative care practices.

      Promote Quality & Professional Excellence

      • Support quality improvement, patient safety, and evidence-based practice initiatives.

      • Participate in professional development, continuing education, and competency maintenance.

      • Serve as a mentor and preceptor for students, advanced practice providers, and clinical staff when appropriate.

      Education

      Required

      • Master's Degree from an accredited Nurse Practitioner or Physician Assistant program

      Licensure & Certifications

      Nurse Practitioner Candidates

      • Current Louisiana Registered Nurse (RN) License

      • Current Louisiana Advanced Practice Registered Nurse (APRN) License

      • National Nurse Practitioner Certification

      • Basic Life Support (BLS) Certification through the American Heart Association

      • Advanced Cardiac Life Support (ACLS) Certification through the American Heart Association

      Physician Assistant Candidates

      • Current Louisiana Physician Assistant License

      • National PA Certification

      • Basic Life Support (BLS) Certification through the American Heart Association

      • Advanced Cardiac Life Support (ACLS) Certification through the American Heart Association

      Experience

      Required

      • Previous Nurse Practitioner or Physician Assistant clinical experience preferred.

      Preferred

      • 3+ years of Palliative Care experience

      • Experience in hospice, oncology, internal medicine, geriatrics, critical care, hospital medicine, or chronic disease management.

      • Experience working with medically complex patient populations.
