Wound care is one of the most clinically engaging specialties in post-acute medicine. Every patient presents differently, every wound tells a story, and the progress you drive is visible and measurable. As an NP or PA with Innovative Wound Solutions, you will manage your own panel of wound care patients across assigned skilled nursing facilities and in private home settings, working with autonomy while staying connected to a collaborative, physician-supported team.
No two days look the same. You will move between facility and home environments, bringing specialized wound care directly to patients who would otherwise go without it. This role is ideal for a provider who wants to develop or deepen a specialty focus, work a structured schedule, and make a tangible difference in patients who genuinely need an advocate.
Wound Assessment and Treatment — Perform comprehensive wound assessments including tissue viability, wound dimensions, infection status, and associated symptoms across both facility and home settings, and implement evidence-based treatment plans tailored to each patient.
Advanced Procedures — Perform wound debridement, apply advanced dressings, and carry out other wound care procedures within your scope of practice in skilled nursing facilities and in patients' homes.
Patient and Family Education — Guide patients and families through wound care techniques, prevention strategies, and self-care practices that support healing and reduce recurrence, with particular attention to home care instruction for patients seen in their own environment.
Collaborative Care — Partner with physicians, nursing staff, physical therapists, home health agencies, and the broader interdisciplinary team to coordinate care and optimize patient outcomes across all care settings.
Documentation and Quality — Maintain accurate, timely clinical records for both facility and home visits, and actively participate in quality improvement efforts that strengthen the care we deliver.
Qualifications
Education — Master's degree or higher in Nursing (NP) or completion of an accredited PA program. Board certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care NP (AGPCNP), or PA-C required.
Licensure — Active, unrestricted California NP or PA license and current DEA registration. APRN certification where applicable.
Experience — Minimum of 6 months of clinical experience as an NP or PA. Prior acute care, hospital, post-acute, home health, or wound care experience is strongly preferred though candidates with strong clinical foundations are encouraged to apply.
Wound Care Certification — WOCN certification or wound care specialization is a plus, not a requirement. We support and encourage certification for the right candidate.
Clinical Acumen — Ability to assess and manage complex wounds across a diverse patient population with strong critical thinking and sound clinical judgment in both structured and independent care environments.
Communication — Clear, compassionate communicator with patients, families, facility staff, home health teams, and care team members across multiple settings.
Adaptability — Comfortable working across varied environments, from skilled nursing facilities to private residences, with the organizational skills to manage a mobile patient panel effectively.
