HarmonyCares is seeking a full-time Nurse Practitioner or Physician Assistant to provide primary care to patients in their homes in and around Hudson County, NJ, including the Jersey City and surrounding areas.

More Time with Patients

With visits typically lasting 30–60 minutes, you'll have the opportunity to conduct thorough assessments, manage chronic conditions, coordinate care, and address the factors affecting each patient's health.

Our compensation model includes a competitive base salary plus an annual bonus opportunity based on quality. Your success is measured by the care you deliver and the outcomes you help achieve, rather than simply the number of patients you see.

Local Patient Visits & Mileage Reimbursement

You'll care for patients within an assigned territory, with visits geographically clustered to help minimize unnecessary travel. Mileage reimbursement is provided for eligible work-related travel.

Clinical Autonomy with Team Support

You'll have the independence to manage your patients' care while collaborating with physicians, fellow APPs, clinical support teams, and operations partners. Technology, including an AI-supported documentation solution, helps streamline administrative responsibilities.

Caring for patients in their homes offers a unique perspective on their health, living conditions, medication management, and access to care. You'll be able to identify challenges that may go unnoticed in a traditional office setting and develop individualized care plans that address the whole patient.

Position Highlights

• Full-time, Monday–Friday schedule
• Established patient panel in Hudson County, NJ area
• Smaller patient panels and manageable daily visit volume
• Comprehensive primary care for medically complex patients
• Longer patient visits, typically 30–60 minutes

• Limited after-hours telephonic call, shared among providers

Qualifications

• Current New Jersey Nurse Practitioner or Physician Assistant license
• Appropriate national board certification
• DEA registration or ability to obtain
• Prior clinical experience practicing as an NP or PA; new graduates are not eligible for this position
• Primary care, internal medicine, family medicine, geriatrics, or related experience preferred
• Ability to manage complex chronic conditions and provide comprehensive primary care
• Comfortable traveling locally to patients' homes within the assigned territory
