Job Description
Matrix Medical Network seeks a Nurse Practitioner to deliver high-quality in-home and community-based health assessments and primary care services. In this role, you will perform comprehensive evaluations, manage chronic conditions, order and interpret diagnostics, and develop evidence-based care plans that support improved outcomes and reduced costs. You’ll collaborate closely with an interdisciplinary team and health plan partners to support quality, risk adjustment, and population health initiatives.
