The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

What You’ll Do
Provide primary medical care for participants under physician supervision
Conduct comprehensive and interval history and exams.
Diagnose and treat episodic illnesses.
Partner with the IDT to coordinate 24-hour, wraparound care.
Participate actively in care planning, documentation, and communication.
Write medical orders and manage consultations to ensure continuity of care.
Support clinic operations across locations and contribute to a culture of excellence.
What Makes You a Great Fit
Master’s in Nursing and Nurse Practitioner degree from an accredited program
Current in RN and NP licensure the State of Michigan
Minimum 1 year experience working with frail or elderly adults
Ability to work both independently and collaboratively in a team setting
Excellent written and verbal communication skills
Proficiency in electronic medical records and MS Office
Demonstrated alignment with our core values: Compassion, Integrity, Teamwork, and Excellence
Current CPR certification (or willingness to obtain)
Reliable transportation and valid Michigan driver's license

Must be fully vaccinated or willing to become vaccinated against:
Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B
Must meet CMS competency standards prior to working independently
Must be willing to work beyond normal hours and take part in our on-call rotation
