A leading integrated care organization is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to support its Senior Community Care (SCC) program. This program delivers longitudinal, facility‑based primary care to high‑risk health plan members across skilled nursing and rehabilitation centers.
The APC functions as part of an interdisciplinary team, providing comprehensive assessments, chronic disease management, advanced illness conversations, and patient/caregiver education. This role offers excellent work‑life balance with no nights, no weekends, no holidays, and no call.

Conduct comprehensive patient assessments in facility settings
Manage acute and chronic medical and behavioral health conditions
Ensure accurate and complete documentation, including ICD‑10 coding
Collaborate with an interdisciplinary care team
Lead advanced illness and advanced care planning discussions
Provide education and counseling to patients and caregivers
Navigate varying facility environments and perform hands‑on physical assessments
Active and unrestricted NP or PA license (or ability to obtain by start date)
Graduate of an accredited NP (MSN/DNP) or PA program (Master’s required)
National board certification:
NP: AANP or ANCC (FNP, ANP, GNP, AGACNP)
PA: NCCPA
Active DEA licensure/prescriptive authority
Ability to lift a 30‑lb medical bag, navigate stairs, and work across multiple facility environments
Reliable transportation
Ability to obtain a collaborative practice agreement (if required by state)
Full COVID‑19 vaccination (per federal, state, and facility requirements)
Ability to pass a pre‑employment drug screen
1+ year of clinical experience (long‑term care strongly preferred)
Experience managing patients with complex behavioral, social, and functional needs
Strong understanding of geriatrics, chronic illness, and end‑of‑life care
Proficiency with EMR systems and electronic documentation
