    Per Diem Nurse Practitioner/Physician Associate - Senior Health (Nursing Home Coverage)

    Elliot Health System's Senior Health Practice is seeking an experienced Nurse Practitioner (APRN) or Physician Associate (PA-C) to join our team on a per diem basis providing care in skilled nursing facilities and long-term care facilities communities throughout Manchester and Bedford, New Hampshire.

    This role provides flexibility while allowing providers to practice meaningful, relationship-based geriatric medicine. The position is primarily focused on nursing home care, with occasional opportunities to support our outpatient Senior Health clinic for acute patient needs.

    This is an excellent opportunity for an experienced provider who enjoys caring for older adults and values autonomy, collaboration, and a predictable schedule.

    Position Highlights

    • Per diem schedule
    • Primary coverage of nursing homes and senior living facilities in Manchester and Bedford
    • Occasional coverage of the Senior Health outpatient clinic for acute visits
    • No patient panel responsibilities
    • No call requirements
    • Collaborative interdisciplinary care team

    Responsibilities

    • Provide comprehensive medical care to geriatric patients residing in skilled nursing, rehabilitation, and long-term care settings
    • Assess, diagnose, and manage acute and chronic medical conditions
    • Perform routine follow-up visits, discharges, and transitional care management
    • Collaborate closely with physicians, nursing staff, rehabilitation services, social workers, and family members
    • Document care accurately and timely in the electronic medical record
    • Participate in quality improvement and patient-centered care initiatives
    • Provide outpatient Senior Health clinic coverage as needed for acute patient care needs

    Qualifications

    Required:

    • Graduate of an accredited Nurse Practitioner or Physician Associate program
    • Current New Hampshire APRN or PA-C licensure (or eligibility)
    • DEA certification (or ability to obtain)
    • Minimum of2 years of experience caring for geriatric patients
    • Strong clinical assessment and decision-making skills
    • Ability to work independently in a nursing home environment while collaborating with an interdisciplinary team

    Preferred:

    • Long-term care, skilled nursing facility, geriatrics, internal medicine, family medicine, palliative care, or post-acute care experience
    • Experience using Epic or similar electronic medical record systems
