Seeking a Compassionate Primary Care Nurse Practitioner in Pell City, AL

Summary: Our client is seeking a experienced andcompassionate Nurse Practitioner or PhysicianAssistant to join their team in delivering high-quality care to our diverseand underserved patient population. The ideal candidate will diagnose, educate,and initiate treatment in an outpatient clinic setting, collaborating with amulti-disciplinary team to promote health and prevent disease. This positionoffers the opportunity to make a meaningful impact in a health center committedto comprehensive, patient-centered care.

Core Responsibilities:

  • ComprehensiveClinical Care: Provide comprehensive primary care services to patients of all backgrounds, including preventive, wellness, and acute care visits.Diagnose, treat, and manage acute and chronic conditions using evidence-based guidelines.
  • Team-BasedCare & Coordination: Collaborate with nurses, care coordinators, behavioral health providers, and specialists to manage care transitions, ensuring continuity, particularly for medically complex patients.
  • PatientAdvocacy & Communication: Build trusting, long-term relationships through effective, empathetic communication. Educate patients and families on disease management and healthy lifestyle changes.
  • EfficientWorkflow & Technology Adoption: Utilize Electronic Health Record (EHR) systems to maintain accurate, up-to-date, and detailed documentation in the electronic medical record system.

  • Patient Base: Senior/ Adult Focused

  • Schedule: Monday–Friday

  • Care Team: Comprehensive support (Nurses, Care Coordinators)

Requirements

Qualifications:

  • Graduate of an accredited NP or PA program
  • Current unrestricted APRN or PA license
  • National board certification & BLS certification
  • DEA registration​
  • Interest in value-based care and population health
