Job Overview

We are seeking an experienced Nurse Practitioner for a flexible, part-time opportunity serving patients throughout Charlottesville and surrounding communities. The position focuses on delivering primary care and post-hospital follow-up services in patient homes and senior living settings.

Coverage is needed 2-3 days per week. Consideration will also be given to two providers who can each work 1-2 days per week, providing additional scheduling flexibility.

Role Overview

The Nurse Practitioner will provide primary and transitional care to patients who require ongoing medical management or follow-up after hospitalization. Responsibilities include assessing acute and chronic conditions, completing post-discharge evaluations, developing treatment plans, and coordinating care with patients, caregivers, and other healthcare professionals.

Key Responsibilities

  • Deliver primary care and post-hospital follow-up services in patient homes and senior living communities
  • Perform comprehensive health assessments and follow-up evaluations
  • Conduct post-discharge visits to support safe transitions from hospital to home
  • Evaluate, diagnose, and manage acute and chronic medical conditions
  • Develop individualized treatment and care plans based on patient needs
  • Prescribe medications and order appropriate diagnostic testing
  • Coordinate referrals and collaborate with interdisciplinary healthcare professionals
  • Educate patients and caregivers on medications, disease management, preventive care, and treatment plans
  • Monitor patient progress and modify treatment plans as clinically appropriate
  • Complete EMR documentation within 24 hours of patient encounters
  • Consult with the Medical Director or collaborating providers when clinically indicated

Schedule

  • Part-time coverage of 2-3 days per week
  • Open to two providers working 1-2 days per week each
  • Flexible scheduling
  • Typical hours: 8:00 AM-5:00 PM
  • No on-call responsibilities
  • Orientation and onboarding provided
  • Credentialing typically takes approximately 30 days

Required Qualifications

  • Active Virginia Nurse Practitioner license
  • Current DEA registration
  • Active CAQH profile and CAQH number
  • Minimum of one year of Nurse Practitioner experience in primary care, family medicine, senior care, assisted living, long-term care, or transitional care
  • Experience using electronic medical record (EMR) systems
  • Strong clinical assessment abilities with the capacity to make independent clinical decisions

Preferred Qualifications

  • Previous post-acute or transitional care experience
  • Clinical background in urgent care, emergency medicine, hospital medicine, or acute care
  • Experience caring for geriatric patients and individuals with complex medical conditions
  • Previous experience delivering care in patient homes or senior living environments
