Job Overview
We are seeking an experienced Nurse Practitioner for an immediate part-time need serving patients in Culpeper, Virginia, and surrounding communities. This position provides primary care and post-hospital follow-up services to patients in their homes and senior living settings.
The ideal setup is a single Nurse Practitioner working 1–2 days per week, with the option to expand to 2–3 days to help cover additional service areas in Winchester, VA.

Role Overview
The Nurse Practitioner will manage patients who need ongoing primary care or clinical follow-up after a hospital stay. The role includes evaluating acute and chronic conditions, completing post-discharge assessments, developing treatment plans, and coordinating care with patients, caregivers, and other healthcare professionals.

Key Responsibilities

  • Provide primary care and post-hospital follow-up services in patient homes and senior living settings
  • Complete comprehensive clinical assessments and follow-up evaluations
  • Conduct post-discharge visits and help patients transition safely from hospital care
  • Assess, diagnose, and manage acute and chronic health conditions
  • Create individualized treatment plans based on each patient's clinical needs
  • Prescribe medications and order appropriate diagnostic tests within scope of practice
  • Coordinate referrals and collaborate with physicians and other healthcare professionals
  • Educate patients and caregivers regarding medications, treatment plans, preventive care, and disease management
  • Monitor clinical progress and adjust treatment plans when appropriate
  • Complete EMR documentation within 24 hours of patient encounters
  • Consult with the Medical Director or collaborating clinicians when additional clinical guidance is appropriate
Schedule
  • Part-time coverage needed 1-2 days per week
  • Flexible scheduling
  • Typical working hours: 8:00 AM-5:00 PM
  • No on-call responsibilities
  • Orientation and onboarding provided
  • Credentialing generally takes approximately 30 days

Required Qualifications
  • Active Virginia Nurse Practitioner license
  • Current DEA registration
  • Active CAQH profile and CAQH number
  • At least one year of Nurse Practitioner experience in primary care, family medicine, senior care, assisted living, long-term care, or transitional care
  • Experience working with electronic medical record systems
  • Strong assessment skills and the ability to make sound, independent clinical decisions
Preferred Qualifications
  • Experience in post-acute or transitional care
  • Background in urgent care, emergency medicine, hospital medicine, or acute care
  • Experience treating geriatric patients or individuals with complex medical needs
  • Previous experience providing clinical care in patient homes or senior living environments
