Are you a Nurse Practitioner interested in providing personalized primary care to patients in their homes and senior living settings? This part-time opportunity in the Tappahannock and Northern Neck area offers a flexible three-day schedule, significant provider autonomy, and the opportunity to build meaningful relationships with patients in community-based environments.

Role Overview
The Nurse Practitioner will provide comprehensive primary care primarily through in-home visits, with additional visits to assisted living and long-term care communities. Responsibilities include evaluating patients, developing and managing treatment plans, conducting post-discharge assessments, and coordinating services to support continuity of care.

Key Responsibilities
  • Deliver approximately 85% of patient care through home visits and 15% in senior care settings.
  • Conduct comprehensive assessments, follow-up visits, and ongoing patient management.
  • Diagnose acute and chronic conditions and create individualized treatment plans.
  • Prescribe medications and order appropriate laboratory, imaging, and diagnostic services.
  • Complete post-discharge evaluations and support continuity of care.
  • Coordinate referrals and work with scheduling staff to organize daily patient visits.
  • Educate patients and families regarding treatment, medications, preventive care, and health management.
  • Monitor clinical progress and modify treatment plans when appropriate.
  • Complete required EMR documentation within 24 hours of each patient encounter.
  • Consult with the Medical Director when additional clinical guidance is appropriate.
Schedule
  • Part-time schedule of 3 days per week.
  • Flexibility to establish your own schedule.
  • Typical working hours are 8:00 AM - 5:00 PM.
  • No on-call responsibilities.
  • Two-week orientation program.
  • Credentialing may require up to 30 days.
  • Optional shadowing opportunities are available.
Service Area
  • Essex, King and Queen, Richmond County, Northumberland, Westmoreland, Lancaster, Middlesex
  • Travel may extend up to approximately 40 miles within the service area.
Required Qualifications
  • Active Virginia Nurse Practitioner license.
  • Current DEA license.
  • Active CAQH profile and CAQH number.
  • At least 1 year of Nurse Practitioner experience in primary care, family care, senior care, assisted living, or long-term care.
  • Experience documenting patient care in an electronic medical record system.
  • Ability and willingness to travel throughout the Tappahannock/Northern Neck service area.
Preferred Qualifications
  • Previous experience in urgent care, emergency medicine, trauma, or inpatient acute care is preferred.
