Matrix Medical Network is seeking a compassionate and adaptable Nurse Practitioner (PRN) to provide high-quality in-home clinical assessments and health management services for health plan members and government-sponsored program participants. In this PRN role, you will travel to patients’ residences or community-based settings to perform comprehensive health evaluations, chronic disease assessments, medication reviews, and preventive screenings. You will document findings using electronic medical records, communicate care needs clearly to health plans and primary care providers, and help connect patients with appropriate resources to improve their overall health and reduce avoidable hospitalizations.

As a Nurse Practitioner with Matrix Medical Network, you will apply your advanced clinical judgment to identify risks, close care gaps, and support quality and compliance standards for our partner health plans. You will collaborate closely with a multidisciplinary healthcare team, including physicians, nurses, and care coordinators, to ensure continuity of care. This position is ideal for a clinician who values autonomy, enjoys working one-on-one with patients in their own environment, and is committed to evidence-based practice and patient education.

You will conduct focused and comprehensive physical exams, obtain detailed medical histories, and interpret diagnostic information available at the point of care. You will educate patients and caregivers on chronic disease self-management, medication adherence, lifestyle modifications, and preventive services in alignment with clinical guidelines. You will also help drive improvements in quality metrics and member satisfaction by carefully documenting assessments, following standardized protocols, and adhering to regulatory and payer-specific requirements.
